Your registration status will be placed on hold. This means that you will not be allowed to register for new classes until we receive your completed form.

You have six weeks from the date of your acceptance letter to submit your 2024-2025 New Student Vaccination and Tuberculin form. Please be prepared to show us your acceptance letter if we have any questions.
What additional forms do I need to fill out to play varsity sports at MIT.
All varsity athletes must complete the Varsity Student Athlete Physical Examination form There are no exceptions. Athletes must have a physical within six months of their sports start date.
