The answer to this question depends what part of your medical record you need to access:

  • Medical record
  • Immunization record only
  • Dental record

How can I view my medical record at MIT Medical?
  1. Call 617-253-4906 to schedule an appointment with the Medical Records Service.
  2. When you arrive for your appointment, you will be asked to present two forms of identification. One must be a government-issued ID such as a driver’s license, state-issued photo ID card, or passport.
  3. You will be asked to sign a logbook.
  4. In the presence of a Medical Records employee, you will be allowed to review your electronic medical record.
  5. You may copy information from your electronic medical record by hand, but you may not take pictures of the screen.
How can I get a copy of my medical record?

To get a copy of your medical record, you must complete and sign the Authorization for Disclosure of Patient Health Care and Information form and fax or mail it to the Medical Records Service as instructed on the form. Copies of your record will be sent via FedEx within 2 to 3 weeks. There may be a fee for release of your medical record. Please read the instructions on the form. 

If you only need a copy of your immunization records, let the medical records staff know.

You generally do not need to get a printed copy of your entire medical record if you:

  • Need to be excused from a class for exam because of illness. In such cases, visit Student Support Services, 77 Massachusetts Avenue, 5-104, or call them at 617-253-4861.
  • Want your parents to be involved in your care decisions.
  • Are traveling overseas and need a copy of your immunization record.
What should I do if I find something in my medical record that I think needs to be changed or corrected?
  • If you need to change or correct your name, date of birth, or gender, please use this form.
  • If you feel that specific medical information in your record is incorrect:
  1. Download and complete the Request for Amendment of Protected Health Information form.
  2. Specify the information you want changed, and sign the form.
  3. Drop off the form in person, fax it to 617-258-0884, or mail it to MIT Medical Medical Records Service, E23-023, 77 Massachusetts Ave., Cambridge, MA 02139.
  4. Medical Records will notify you when a decision is made about amending your record.
How can I get a copy of my immunization record?

If you need a copy of your immunization record for international travel or for another reason, fill out the Authorization for Release of Protected Health Information (Medical Record) by MIT Medical and indicate that you only need a copy of your immunization record. It should be available within two business days.

How can I have my medical record from another provider sent to MIT Medical?
  1. Download and complete the Authorization for Release of Protected Health Information (Medical Record) to MIT Medical.
  2. Send the form to the doctor or facility from whom you wish to obtain your medical record.
How can I get a copy of my MIT dental record?
  1. Download and complete the Authorization for Use and Disclosure of Protected Health Information – Dental Record.
  2. Fax the form to 617-258-5500, or mail it to MIT Medical's Dental Service, Room E23-530, 77 Massachusetts Ave., Cambridge, MA 02139.
How can I see my MIT mental health record?

You can make an appointment to view your mental health record by calling Student Mental Health and Counseling Services at 617-253-2916.

How can I get a copy of my MIT mental health record?
  1. Download and complete the Authorization for Release of Protected Health Information (Mental Health Record) by MIT Medical.
  2. Fax the form to 617-253-0162, or mail it to the Mental Health and Counseling Service, MIT Medical E23-364, 77 Massachusetts Ave. Cambridge, MA 02139.
How can I have another provider send my mental health record to MIT?
  1. Download and complete the Authorization for Release of Protected Health Information (Medical Record) to MIT Medical.
  2. Send it to the doctor or facility from whom you wish to obtain your medical record.
What should I do if I find something in my mental health record that I think needs to be changed or corrected?
  • If you need to change or correct your name, date of birth, or gender, please use this form.
