What’s this new tax form?

MIT students and affiliates covered by the MIT Student or Affiliate Extended Insurance Plans and Whitehead employees enrolled in the MIT Traditional Health Plan will find a new “Important Tax Document”—a form 1095-B—in their mailboxes for the first time this tax season. This form provides proof that you had health insurance meeting “minimal essential coverage” requirements under the Affordable Care Act (ObamaCare) during the 2015 tax year. You may receive this form even if you don’t file U.S. tax returns. But if you do file a federal tax return, the government’s health insurance website tells you not to include the 1095-B form with your tax return. If you or your family were covered by one of the above MIT health plans (or one of the above plans plus another qualifying insurance plan) for the full year, you should simply check off the “full-year coverage” box on your tax form and save the 1095-B form for your records. 
 

As in previous years, students, affiliates, and employees will also receive a 1099 HC form from Blue Cross Blue Shield. This form should be filed with your Massachusetts state tax form. 

MIT students, affiliates, and Whitehead employees will receive their forms 1095-B from the MIT Health Plans Office. If you are a student or affiliate with a question about this form, you may call 617-253-2558 or send an email to 1095B@med.mit.edu.

MIT employees will receive forms 1095-C—“proof of minimal essential coverage”—from MIT Human Resources. Employees with questions about this form should call 617-253-6151 or send an email to benefits@mit.edu.
