Student Medical Report Form

All incoming students must complete the Student Medical Report Form, with no exceptions. If you will be enrolling at MIT for the spring term, your deadline to submit this form is January 20 — or, if you were accepted late, six weeks from the date on your acceptance letter.

If you fail to meet the deadline for submitting this form, your registration status will be placed on hold. This means that you will not be able to register for new classes until we receive your completed form. So, if you arrive as a freshman in the fall, but do not submit your medical report form, you will not be allowed to register for your spring-term classes.

If you need to complete certain requirements after arriving at school—for example, if you need the final dose of the three-dose Hepatitis B vaccine or if you are missing any other requirement—be aware that the MIT Student Health Insurance Plan (MIT SHIP) does not cover immunizations or tests that should have been completed before your arrival at MIT. You can get that last Hepatitis B shot at MIT Medical, but you will have to pay for it yourself.
