All information must be received before the following deadlines to avoid a registration hold.
Term:
- Summer: May 3, 2024
- Fall: July 19, 2024
- Spring: January 17, 2025
All new students must:
Complete the 2024-2025 New Student Vaccination and Tuberculin form. Your proof of vaccinations and/or required laboratory reports must be uploaded when you submit the form.
Varsity student-athletes must:
Complete the Varsity Student-Athlete Physical Examination form in addition to submitting the 2024-2025 New Student Vaccination and Tuberculin form
Health Science and Technology (HST) students must:
Submit the following documents:
- proof of vaccinations including positive titer results for the following: measles, mumps, rubella, hepatitis B, and varicella
- tuberculosis screening test (either ppd or IGRA) done within six months of enrollment
